1. Services

UnityCare provides IT services for healthcare, manufacturing, legal and other organizations, including cybersecurity, network infrastructure, helpdesk support, cloud solutions, EMR/EHR integration, managed IT services, website design and hosting, and eFax services. Our service offerings and their descriptions are available on our website. Specific services are governed by the written agreement we sign with each client.

2. User Responsibilities

You agree to:

• Provide accurate and complete information when communicating with us or using our services
• Maintain the confidentiality of any account information, including passwords, and promptly notify us of any unauthorized access or use of your account
• Use our website and services only for lawful purposes and in accordance with these Terms
• Not use our website or services in any way that could damage, disable, overburden or impair them
• Not send protected health information (patient or resident information) through this website, by text message, by email, or in a phone or voicemail message unless we have arranged a secure method with you

3. SMS Messaging Terms

UnityCare may send service-related text messages (SMS) under the program name UnityCare IT Alerts to people who have opted in. These messages include support ticket updates, appointment reminders and critical service notifications. We do not send marketing or promotional content via SMS.

• Opt-in: You may consent to receive SMS messages by texting START to 405-285-3845, by checking the SMS consent box on our SMS opt-in page (unitycareit.com/sms-opt-in), or by requesting SMS support by email or phone. By opting in, you agree to receive informational text messages related to UnityCare services.
• Opt-out: Reply STOP at any time. You will receive one confirmation message and no further texts will be sent unless you opt in again.
• Help: Reply HELP to any message, or contact us at start@unitycareit.com or 405-285-3845 (call or text).
• Message frequency: Message frequency varies depending on your service activity.
• Fees: Message and data rates may apply. UnityCare does not charge for SMS messages.
• Eligibility: You must be 18 or older and the authorized mobile account holder (or authorized to enroll the number). Service is limited to U.S. mobile numbers and may not be available on all carriers.
• Data and privacy: We collect your mobile number, your consent and the date, time and method of your opt-in, and our messaging provider keeps a history of messages, solely to operate the program. This information is shared only with our SMS service provider and wireless carriers for message delivery. Mobile numbers and SMS opt-in data are not shared with third parties or affiliates for marketing purposes. Text-messaging originator opt-in data and consent will not be shared with any third parties. • Carriers: Carriers are not liable for delayed or undelivered messages.

These Terms are governed by the laws of the State of Oklahoma, without regard to conflict of law principles. Disputes shall be resolved in the state or federal courts located in Oklahoma County, Oklahoma.
