New Hire IT Onboarding Checklist for Care Facilities

New nurses and aides arrive eager to work, and often the first thing they hear is that their login is not ready. The scheduler is waiting on HR, HR is waiting on IT and IT never got the request. Meanwhile, a unit is short-staffed and a supervisor is escorting someone around on a borrowed account.

A repeatable onboarding checklist solves this and protects security at the same time. Giving the right person the right access, and only that access, is a core HIPAA safeguard.

Start with a trigger

The checklist starts when HR or the department manager submits a request, ideally at least a few business days before the start date. Use one form, not scattered emails. It should capture:

Name, role, department and unit

Start date and supervisor

Employee, contractor or agency status and planned end date

Systems needed, based on role

Equipment needed

Whether the person will work remotely

Role-based access

Define standard access packages by job title instead of copying a coworker's permissions. For example:

Nurse: EHR clinical access, scheduling, secure messaging, unit printers

CNA or aide: limited EHR documentation, time clock, communication tools

Business office: billing modules, finance systems, shared drives

Administrator: broader access with stronger authentication

The HIPAA minimum necessary standard applies here: staff should have access to the information they need to do their jobs and no more. Role packages make this easy to apply and easy to audit.

Accounts and credentials

Create individual accounts. Never share logins, even temporarily.

Enforce multi-factor authentication from day one.

Deliver initial passwords securely, not in a plain email, and require a change on first sign-in.

Set up email, phone extension, badge or door access as needed.

Add the person to the right groups and distribution lists.

Equipment

Assign and record:

Laptop, desktop or shared workstation access

Mobile device or tablet if used

Headset, scanner or badge reader

Phone and voicemail setup

Log the serial number and who it was issued to. Make sure devices are encrypted, patched and have security software before handing them over.

Day-one training

A short IT orientation, around 20 to 30 minutes, covers more than most organizations expect to need:

How to sign in, lock the screen and use MFA

How to reach the helpdesk and which channel to use for urgent issues

How to spot and report phishing

Rules for personal devices, texting and photographing residents or screens

Where to store files and what not to send by regular email

Acceptable use and confidentiality acknowledgment

Have the new hire sign the policies, and keep the record for compliance.

Agency and temporary staff

These workers need access quickly and are often forgotten at offboarding. Set an automatic expiration date on their accounts that matches their contract, and require supervisors to extend it deliberately. Limit them to the systems they need, and provide the same training in a shortened form.

Check on day one

Have someone confirm that the new hire can actually sign in to each system before the shift begins. A quick call from the helpdesk on the first morning catches problems early.

Do not forget offboarding

Onboarding and offboarding should mirror each other. When someone leaves, disable their accounts promptly, collect equipment, remove them from groups, forward or archive mail according to policy and update your inventory. Delays here are a common audit finding.

Keep the checklist current

Review the checklist every six months with HR and department managers. New systems, changed job titles and updated policies quietly make old checklists wrong. Ask recent hires what was missing on their first day, since they will point out gaps faster than anyone. A checklist that reflects reality is used; one that is out of date gets ignored.

Measure the process

Track how long it takes from request to a fully working account, how many new hires have problems on day one and how many accounts remain active after departure. These numbers show whether your process is working.
