Onboarding a New Nurse in Under a Day: The IT Checklist

When a new nurse shows up for orientation and cannot log in, everyone feels it. The nurse is frustrated, the preceptor is distracted, and someone is borrowing a coworker's credentials, which creates compliance risk. The fix is a repeatable onboarding process that starts days before the first shift and involves HR, clinical leadership and IT.

Here is a practical checklist you can adapt for nurses and other clinical staff.

Confirm the hire and start date, and send IT a new hire request at least several business days in advance

Specify role, department, unit, shift, supervisor and any similar user to model access after

Note whether the person works across multiple facilities or will need remote access

Confirm license and credential verification are complete according to your policy

Create the user account with a unique username, never a shared login

Assign an email address and licenses for needed applications

Apply role-based access, so the nurse sees what their job requires and no more. HIPAA's minimum necessary standard supports this approach.

Create the EMR account with the correct role, such as LPN, RN or charge nurse, and the appropriate unit assignments

Prepare a temporary password using a secure delivery method

Assign or configure equipment such as badge, laptop or tablet if used

Set up multi-factor authentication enrollment for the first day

Walk through the first sign-in, including setting a new password and enrolling in MFA

Confirm the nurse can reach the EMR, medication administration record, time clock and email

Test printing and any scanners or badge readers on their unit

Provide a one-page quick reference with the helpdesk phone number, portal link and what to do in an outage

Security basics

Keep this brief and practical, about fifteen to twenty minutes.

Never share your password or badge

Lock the screen whenever you step away

Recognize phishing and report suspicious email right away

Do not text resident information through unapproved apps

Where to find the acceptable use and privacy policies, and the acknowledgment form to sign

Training is required under the HIPAA Security Rule, and documenting attendance supports your compliance file.

Introduce the helpdesk

Let the nurse know how to reach support, what information to include in a ticket and who to call if a problem blocks care. Giving them a quick win, such as submitting a test ticket, builds confidence.

First week

Check in after day three to see if anything is missing or slow

Confirm access is correct and fix gaps without sharing logins

Verify the nurse completed security awareness training

Have the preceptor confirm no workarounds are in use

Keep the process measurable

Track a few simple numbers:

How many hires had everything ready on day one

How many days of notice HR provided

The most common missing items

If late notice is the problem, talk with HR about building the IT request into the offer letter workflow.

Do not forget the other end

Onboarding and offboarding are two halves of one process. Use the same request form to record role changes, transfers and departures, so access reflects each person's current job. Same-day removal of access when someone leaves is a critical safeguard.

Common pitfalls

IT learns about the hire the day they arrive

Accounts are copied from a coworker, carrying over excess access

Temporary passwords are sent by email or text in plain form

MFA enrollment is postponed, leaving the account protected only by a password

Contract and agency nurses are overlooked, receiving shared logins

Agency and float staff deserve the same process, with expiration dates on their accounts.

Make it routine

A short template that HR completes for every hire often solves most problems. Include fields for role, supervisor, start date, equipment and systems needed. Review it quarterly with IT and clinical leadership.
